Our office consists of Dr. Gary B. Kaplan, Dr. Russell A. Blair, Dr. Andre M. Fabien and Lisa Kuhen, CNP.
Our office hours are Monday through Friday, 8:30 AM – 4:00 PM.
Please fill out the forms below and bring them with you to your appointment along with the following:
Insurance card
Photo ID
Medication list
Referral (if required)
Co-Payment is expected at the time of your visit. For your convenience, we accept MC/VISA, Discover, checks, and cash.
Also bring with you, or arrange to have sent to our office, any medical records (chest x-rays / cat scans) from your physicians that will help us in your medical care. Records can be mailed or faxed to the address/fax number listed at the bottom of this page.
We accept most major insurances. Please verify with your insurance company if a referral is required prior to your visit. If a referral is needed, please bring a copy of the approval to your visit.
If you are unable to keep your appointment, a 24 hour cancellation notice is required to avoid a $25.00 charge.
